CaseSnap Privacy Policy
This privacy policy pertains to the use of the software application CaseSnap (“Application”) for mobile devices that was created by PA & Associates Healthcare, LLC (“PAHC”). The Application collects and stores billing information about surgical cases within an online platform. This information may include personal identifiable information (PII) and protected health information (PHI). This policy addresses how PAHC handles personal information that CaseSnap collects and receives.

We use third parties, such as data entry companies, in the delivery of our services. The data collected using the Application will be shared with data entry companies. PAHC does not rent, sell, or share personal information with other people or nonaffiliated companies for promotional purposes. Except as otherwise provided in this policy, PAHC shall not intentionally disclose any personally identifiable information to any third party. We will share your information with third parties only in the ways that are described in this privacy policy.
Security
The security of personal information is important to us. When sensitive information such as protected health information is entered using our Application, we use TLS to securely transmit our data and we encrypt that information using AES. We follow generally accepted industry standards to protect the PII and PHI submitted to us, both during transmission and once we receive it. The Application and methods of data security are compliant with Health Insurance Portability and Accountability Act of 1996 (with such Standards commonly referred to as HIPAA. and The Health Information Technology for Economic and Clinical Health Act included in Division A, Title XIII, Subtitle D of The American Recovery and Reinvestment Act of 2009, Pub. L. No. 111-5, 123 Stat. 115 (February 17, 2009), and any regulations or agency guidance issued pursuant thereto (HITECH.) and federal substance abuse confidentiality laws. No method of transmission over the Internet or method of electronic storage is 100% secure. Therefore, while we strive to use commercially acceptable means to protect PII and PHI, we cannot guarantee its absolute security.
Opt-out Rights
You can stop all collection of information by the Application by uninstalling the Application. You may use the standard uninstall processes which may be available as part of your mobile device or via the mobile application marketplace.
Law and Protection
We may be required to disclose PII and PHI under special circumstances, such as to comply with any valid legal inquiry or process such as a search warrant, subpoena, statute, or court order. We will also release specific information in special cases, such as a physical or property threat to our client or others. It may be necessary to share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of our terms of use, or as otherwise required by law.
We may also disclose user provided and automatically collected information if PAHC is involved in a merger, acquisition or sale of all or a portion of its assets. If this is the case, you will be notified via email and/or a prominent notice on our Web site of any change in ownership or uses of this information, as well as any choices you may have regarding this information.
Updates to Our Policy
We may update this policy at any time and for any reason. If there are significant changes to how we handle PII, we will send a notice to the contact email address specified in the user’s account or by placing a prominent notice on our site. You are advised to consult this privacy policy which resides online regularly for any changes.
Miscellaneous
These terms will be governed by and construed in accordance with the laws of the State of Maryland without regard to any jurisdiction's conflicts of law provisions. Any action at law or in equity that arises out of or relates to these terms will be subject to binding arbitration in accordance with the laws of the State of Maryland. If any of these terms and conditions are found to be unlawful, void, or for any reason unenforceable, then that provision will be considered severable from the remaining terms and conditions, and will not affect the validity and enforceability of the remaining provisions. This is the entire agreement relating to the subject matter it contains. This agreement may be modified only by our posting of changes to these terms and conditions, or by a writing signed by both parties.
Your Consent
By using the Application, you are consenting to our processing of your information as set forth in this privacy policy now and as amended by us through any or all of these methods: the use of cookies on a computer/hand held device or using or touching information in any way, including, but not limited to, collecting, storing, deleting, using, combining and disclosing information, all of which activities will take place in the United States. If you reside outside the United States your information will be transferred, processed and stored there under United States privacy standards.
